Insurance Info

The insurance industry is dynamic and rapidly changing. It is best to directly contact your insurance carrier to confirm participation of Colorado Sports Medicine & Orthopaedics with your individual plan.

Many insurance carriers require that we collect co-pays and/or deductibles from the patient. If you have an office visit co-pay for a specialist, you will be expected to pay that amount at the time of service.

Do I need a Referral?

A referral may be required in order to be seen by a specialist. Your insurance company can tell you if the referral is required. If it is required by your insurer, the referral must be received by our office at least 24 hours prior to your visit or we will need to reschedule to a later date.

Can I Use Worker’s Compensation?

For patients whose injury is work related, you must provide the schedulers with all pertinent information such as insurance carrier, claim number, date of injury, adjustors name, phone and fax number before the appointment is scheduled.
